Guiosvany Hernandez - Kidney Coordinator

As a Kidney Coordinator, I provide renal care support and healthcare resources to the underserved population of Collier County. I work closely alongside physicians, nephrologists, dialysis clinics, and medical staff to help ensure patients receive compassionate, high-quality care throughout their treatment journey. My responsibilities include relaying patient concerns, medical needs, and updates to nephrologists and dialysis centers, coordinating communication between healthcare providers, and assisting patients with transitioning to outpatient dialysis facilities. I also help patients arrange home health services to support their care needs and ensure a smoother, safer transition back home after treatment or hospitalization. Through patient advocacy, care coordination, and resource management, I strive to improve continuity of care and help patients successfully navigate the challenges associated with chronic kidney disease and dialysis treatment.
